1. In consideration of acceptance of my registration into the Jiu Jitsu Phuket program (owned by (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd;), I, for myself, my heirs, my executors, my administrators, my trustees, and any and all successors in interest, fully and forever waive and release any and all rights and claims for any injuries and damages, including but not limited to demands or actions for negligence, premises liability, emotional injury, intentional conduct, resulting in unintended injury or damage tort claims, any other actions or demands of whatsoever nature, to the fullest extent permitted by law, I may have against (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; (6) Future Champions Pty Ltd; and (5) all employees, principals, directors, shareholders, agents, members, managers, affiliates, volunteers, officials, and representatives acting for or on behalf of any of these entities.

2. I acknowledge that I am aware of the inherent risks involved in joining Jiu Jitsu Phuket Co., Ltd. company, and the inherent risks of the sport of Brazilian Jiu Jitsu, and I voluntarily assume these risks. I attest and verify that I am physically fit and I am medically able to participate in the (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd;  program and that a medical doctor has verified my physical condition.

3. As a condition of my participation, I hereby grant (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd;  a limited license to use my name, likeness, image, voice, video, athletic performance, biographical and other information, in any format whatsoever, and to distribute, broadcast and exhibit these images and the images generated during my participation in Jiu Jitsu Phuket without charge, restriction or liability (collectively, “Image”). 

4. (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd; reserves the right to reject my registration and membership, and further reserves the right to change the details of the company without prior notice.

5. (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd; regularly uses photographs or videos of seminars, classes and members for advertising and promotional materials to promote the company.  I hereby give (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd; their assignees, licensees, and legal representatives and distribution partners the irrevocable right to use their name or fictional name, video,  picture, portrait, and/or photograph in all forms of media and in all manners, including composite or distorted representatives, for advertising, trade, or any other lawful purposes in perpetuity.  I also agree to waive the right to inspect or approve the finished version(s) including written copy that may be created in connection there in. I understand that I have no rights to any profits derived from any advertising using their name or likeness. I also attest that I am at least 18 years of age or accompanied by a guardian and have read this release, and are familiar with its contents.

6. Membership Fees are Non-Refundable. 

7. Hereby declare that I understand the conditions of (1) Jiu Jitsu Phuket Co., Ltd. (2) Gracie Barra; (3) Paul How; (4) Christopher Christou; (5) The Portal Phuket; and (6) Future Champions Pty Ltd; which I have registered for and will abide by the organizers own rules and guidelines.

DO NOT SIGN THIS AGREEMENT UNLESS YOU UNDERSTAND THE TERMS COMPLETELY. IF YOU DO NOT UNDERSTAND, YOU SHOULD SEEK LEGAL COUNSEL.
